Dear all,

Do you have experience in getting a copy of Social Security
application form (the SS-5)?
I understand the fee for this service is USD27.
Do you know if they send it to Portugal? Or there is an
additional tax if requested abroad?

de qualquer forma estou a perguntar se e´ possivel receber
uma copia do SS-5 em Portugal.
