Here is a sample of questionnaire for 1877 translated from the
Russian:

Name, father's name, city, and age.

Are you single?

Have you legal papers?

What is your religion?

Are there any impediments? From relatives.

Are you being coerced into this marriage?

Are you promised to another?

How long have you two been acquainted?

When did you last go to confession or communion?

As noted above we attest to the truth of these statements by our
signature.

Signed by the bride and groom, the marriage witnesses, and the priest

Vera is right. I would call it a pre-nuptial questionnaire.
