Hello Wendy
I can't advise on records existing for the issue of commemorative plaques, but I do believe that records for medals awarded is available at the PRO.
I do have a letter sent to my Great Uncle's parents with his medals from Woking dated 1920 (he was killed in 1918) so it took some time for these things to go through .
they did also receive the memorial plaque.
