Being from Iowa, I'll give you the first hand reason for one instance of the use of a middle name. We have two daughters. The first we named Mary Belinda. It almost seemed a duty to have Mary or Marie someplace in a girls name. We always called her Belinda. She still goes by Belinda. This is mainly because we liked the name Belinda. Yet a name like Bleinda Mary just seemed awkward. So we named her Mary Belinda and used the second name.
My wife also has Mary in her first name. She was always called by her second name because her parents liked it and it avoided confusion with other Marys in her family. When we married, she continued the use of her second name for the same reason. She did not want confussion with other Mary's.
