I think this is Noah and Lillie's mother's death record. It has Jefferson County as the death place. She may have went to Louisville to be put in a hospital and passed away there.I checked Noah's WWI card and for dependants he had his mother, wife and child. This was Jan. 1918. On the 1920 census his mother was not there.
