The "removed" part is easier to understand if you draw a chart. I'm not sure that I can do this easily on this board, but I will try.
Generation 1 - John Smith - common ancestor
Gen. 2 - John Smith, Jr. - Paul Smith siblings
Gen. 3 - Tom Smith - Bill Smith - first cousins
Gen. 4 - Paul Smith - Tim Smith - second cousins
Gen. 5 - Adam Smith - Isaac Smith third cousins
Gen. 6 - Mark Smith - Michael Smith fourth cousins
If you list the generations in succession, you can draw a line straight across between siblings, first cousins, second cousins, etc. If you want to see the relationship between the removed cousins, draw a line or otherwise count down. For example, to see the relationship between Tom Smith and Tim Smith, you would have to count down one row or one generation. That makes them first cousins once removed. To see the relationship between Tom Smith and Isaac Smith, you have to count down two rows or two generations. That makes them first cousins twice removed. Paul Smith and Isaac Smith are second cousins once removed. Tom Smith and Michael Smith would be first cousins three times removed.
