Another thing to keep in mind is that some states have 2 versions of
death certificates (FL and IN come to mind) - one is usually titled
"Certificate of Death" and gives minimal information - date of death &
name and maybe a bit more; the other is the 'long form' and gives cause
and manner of death, informant's, parents' names, etc.
