I am wondering which formed you used. Did you send it to the Muncipal Archives
on Chambers St? There is the death certificate forms but there is also
one for the medical examiner's report. The medical examiners report can
take a long time (I have been waiting since last summer as they are being
indexed and not back yet). Also, in order to obtain the medical examiner's
report, the death would have had to occur prior to 1940.

I have received other death certificates for my family and it usually only
takes about 4 or 5 weeks. I got one for my grandfather who died in 1965
without any problem.

If you have a copy of the form, you might want to check it. If you have
the right one, you need to write them for an explanation. Something doesn't
seem quite right.
