Mike,

If the deaths you are referring to are the 1954 and 1961 dates mentioned
in your previous posting, I think the problem is you are sending to the
wrong place.

Deaths after 1948 are not at the Municipal Archives, but with the Department
of Health - Office of Vital Records. Generally, relatives of the deceased
are entitled to receive certified copies of the death record. In Box #14,
write "Family Records"
