LAWRENCE COUNTY, KENTUCKY Will Book 11, pp. 779-780: “At a Surrogate’s
Court, held in and for the County of Niagara, at the Court House in the City
of Lockport, New York, on the 16th day of September, A.D., 1987.
PRESENT—HON. ALDO L. DiFLORIO, Surrogate. IN THE MATTER OF PROVING THE LAST
WILL AND TESTAMENT of Haskel Pack, Deceased. File No. 58072, 1987.

Carolyn V. Pack, having this day filed in this Court an instrument in
writing purporting to be the Last Will and Testament of Haskel Pack, late of
the Town of Cambria in the County of Niagara and State of New York,
deceased, together with a duly verified petition praying that said
instrument be admitted to probate as and for the Last Will and Testament of
said decedent; and all his distributes having duly appeared or waived the
issue and service upon themselves of a citation herein; and the said
petitioner appearing by A. Angelo DiMillo, Attorney in support of the
probate of the instrument so propounded, and the witnesses to said
instrument having been sworn and examined before a Notary Public authorized
to take such proofs under Section 1406 of the Surrogate’s Court Procedure
Act and their examination having been reduced to writing and filed, and it
appearing by said proofs that said instrument was duly executed as and for
the Last Will and Testament of said Haskel Pack deceased, and that at the
time of executing the same he was of sound mind, memory and understanding,
in all respects competent to devise real estate and not under any restraint
or in any respect incompetent to make a will, and the probate of said
instrument not having been contested;

IT IS ORDERED, ADJUDGED AND DECREED, that the said instrument in writing so
offered for probate is the Last Will abd Testament of the said Haskel Pack
deceased, and was duly executed as required by law; that the same is valid
to pass real and personal property, and should be and hereby is admitted to
probate.

AND IT IS FURTHER ORDERED, ADJUDGED AND DECREED, that Letters Testamentary
issue to Carolyn V. Pack executrix in said Will named, she having appeared
and taken the official oath prescribed by law, and that said Will and this
Decree be recorded as required by law.

To All to Whom These Presents Shall Come or May Concern, Send Greeting:

Know ye, That at the County of Niagara, on the 16th day of September in the
year of our Lord, one thousand nine hundred and Eighty-seven before HON.
ALDO L. DiFLORIO, Surrogate of our said County, the Last Will and Testament
of Haskel Pack deceased, was proved, and is now approved and allowed by us,
and the said Haskel Pack having been at or immediately previous to his death
an inhabitant of the County of Niagara, by reason whereof the proving and
registering of said Will and the granting administration of all and singular
the goods, chattels and credits of the said testator and also the auditing,
allowing and final discharging the account thereof, doth belong unto us, the
administration of all and singular the goods, chattels and credits of the
said deceased and the execution of any and all powers conferred by said Will
are hereby granted unto Carolyn V. Pack, Executrix in the said Will named,
she being first duly sworn faithfully and honestly to discharge the duties
ofsuch Executrix according to law.

IN TESTIMONY WHEREOF, We have caused the seal of office our our said
Surrogate to be hereunto affixed.
