SURVIVING SPOUSE, NEXT OF KIN, LEGATEES AND DEVISES

The following are decedent’s known surviving spouse, and other known
survivors who are or would be entitled to inherit under the statutes of
descent and distribution.

Myrna L. Pack, 696 E. Moler Street, Columbus, OH 43207 Surviving Spouse
Timothy W. Pack, 802 Bruck Street, Columbus, OH 43206 Son

The surviving spouse is the natural or adoptive parent of at least one of
decedent’s children.

The following are known persons suriving decedent, and named in his Will as
legatees or devises.

Timothy W. Pack, 802 Bruck Street, Columbus, OH 43206 Son

The Will is not subject to Revised Code Sections 109.23 to 109.41, relating
to charitable trusts.

ENTRY APPOINTING FIDUCIARY; LETTERS OF AUTHORITY

Name and Title of Fiduciary – Myrna L. Pack, Executor

On hearing in open court the application of the above fiduciary for
authority to administer decedent’s estate, the Court finds that:

Decedent died in testate on 9-20-92 domililed in Columbus, Franklin County,
Ohio

Bond is dispensed with by the Will.

Applicant is a suitable and competent person to execute the trust.

The Court therefore appoints applicant as such fiduciary, with the power
conferred by law to administer fully decedent’s estate. This entry of
appointment constitutes the fiduciary’s letters of authority.

and devisees known to the applicant.

The undersigned, being persons entitled to notice of the probate of the
Will, hereby waive such notice. Any action to contest the validity of the
Will must be filed within four months after the fiduciary files an affidavit
with the Court evidencing the giving of this notice.

ENTRY ADMITTING WILL TO PROBATE

The Court finds tht the purported Will of decedent, either on its face or
from the testimony of the witnesses, complies with the applicable law. It
is therefore admitted to probate and ordered recorded. The Court further
orders that notice of the probate be given the fiduciary to all parties
entitled to notice pursuant to R.C. 2107.18 and 2108.19

AFFIDAVIT OF SERVICE OF NOTICE OF PROBATE OF WILL

The undersigned, fiduciary of the within estate, hereby states that all
persons required to receive notice as provided in R.C. 2107.19 (A)(1), have
either been notified of the hearing on the probate of the Will, have waived
notice of the Will having been admitted to probate, or their names or places
of residence are unknown to the fiduciary and cannot with reasonable
diligence be ascertained as indicated on the attached form 1.0. Said
waivers and/or evidence of notification are attached hereto.
