Those applications and certificates sound like something that would be in the military records at the National Archives. I haven't ordered anything from NARA in a very long time and by now I had assumed that ordering can be done online with a credit card. Indeed it can be. Instructions and order forms are many layers down at the website:

but it is possible to order online and pay astronomical fees (from $14.75 to $37.00) by credit card for pension/military files before World War I. And turnaround time starts at 6 weeks. Just keep pointing and clicking until you get to the order form layer of the website.
This doesn't answer your question on the two John Piatts but does let folks know we don't have to fill out those awful forms in triplicate and send them by mail any longer.
