I would put the source as the person doing the report, also. If you receive
copies of the documents, put them in as sources using the professional as
the source for the document.
When I do a job, I always include copies of the documentation with the
report.

I haven't ever used a professional, but I would still source that person as
the source of my information. This is I guess a matter of preference. But I
prefer to let other's know exactly where the information came from. Even if
I hold originals in my hands.
