I don't believe the difference in styles for bibliography entries is
based on the number of citations. As I understand it, it is based on
the nature of the source record. Sources that are "authored," be they
books, articles, letters, or what ever, have bibliography entries
based on the "author." Sources that are public record based, and
likely to be available from a single location, have a bibliography
entry based on that location.
Birth records as covered by the example "Evidence!" are assumed to be
public records, and thus the bibliography template starts with the
repository address, which would that be the county recorder, state
health department, or where ever those records are kept.
It sounds like you, as I have, considered the certificates issued by
those bodies to be the source, rather than a public record at a court
house or whatever. My logic was that I never saw the court house
record, but only saw the certificate in my possession. That approach
requires a different bibliography entry, in my opinion. The one I use,
with little or no basis in Mills, is:
[SUBJECT] [RECORD TYPE], [AGENCY], [ADDRESS].
