I expect that you are speaking mostly of Custom tags. TMG will build the
Tag and Witness Sentences, but as you say, they are often not very good
grammatically. If you aren't sure whether a Tag is Custom or standard,
click on the plus (+) icon on the left of the Person View to display the
Tag Type List. Then click on the More>> button to change to the Expanded
Tag Type List. Then as you highlight any tag, the information about the
tag will be displayed showing whether it is standard or Custom.
On the other hand, the default Sentences of standard Tags are good. You
can change the default Sentences of either standard or Custom Tags either
globally or for any specific tag. If you change a Sentence for a specific
tag and then globally change the Tag or Witness Sentence, the individual
tag modified Sentence will remain.

No, there is no change in how TMG builds Sentences. But don't let that
stop you from getting the v4.0d update. It is worth downloading and won't
cost you anything.

No, you will have to change it yourself. As for v5.0, I doubt there would
be mnuch change in this area even as late as v10.0. <g>
