Assuming the fact of the adoption isn't a secret whose disclosure will cause hurt feelings (in which case you have a whole other issue to deal with) why wouldn't one just list the adopted child, making it very clear that child was adopted? There are a number of possible ways to do this, depending on how much, if anything, you want to say about the birth parents. While TMG offers some ways to do this in reports, if you are preparing "the book" you can edit the wording to refine the wording to best fit the family situation.
