It up to you. You can only have one primary mother you can specify the
stepmother or the natural mother as primary. This is the one that will print
in charts and reports.

e.g. on an ancestory report if you specify the stepmother then the
stepmother's parents will appear as you mother's ancestor.

I decide on a person by person basis which way to handle it. But I have a
Parent-note tag (in the other group) that I use to cover the other parent.

e.g. if I decided to make the natural mother the primary mother I would add
my parent note to read something like.
Her stepmother, xxxx, raised her and was really the only mother she ever
knew.

But it's really up to you which way you want to handle the situation
