Carlton,
I just followed what you had done and think I know what you are missing.
See below.

All good and same thing happened for me. This happens because you have
not yet created the custom source elements for county and state--see my
comments below for how I did this step by step.

Terry does say on his page you are referencing that you will have to
create custom source elements for county and state; he just doesn't give
detailed instructions on how to do it. Here's what I did:
--when you're at the source definition screen, as you've described
above, click on one of the empty box labels where you want to see county
and state appearing
--a Source Elements box will appear--click on Add
--enter County as the label
--then you must put it into a group (see Terry's article
http://www.reigelridge.com/tmg/Sources%20-%20Creating.htm for further
explanation if desired); I put this into the Location group and then
clicked ok
--after saying ok, you should be back at the Source Elements box
again--click Add again and enter State as the label this time
--now you have to select a group for State--you must NOT put it into the
same group as County, since only one element from each group can be
used--thus I put State into the Second Location group
Now when you create a new source using the Census 1930 source type,
those fields should appear. I just did this (using 5.04) and it worked
all right for me. If it doesn't work for you, speak up as I may have
miscommunicated something.
