Doctor etc. would go into the to Title box. Where you place the prefix to
the surname depends a lot on the situation. Often the prefixes mean "of" or
"the" in the native language and thus it can make sense to not sort on them
just like we normally ignore a leading "A" or "The" in a books title. It can
make sense to ignore these when sorting names, particularly if later
generations dropped the prefix. If latter generations merged the prefix then
it could make sense to leave the prefix in the surname field (though they
still won't sort exactly together).
