If the above W refers to a relative or Spouse I doubt that a death
certificate would mention who was in attendance at the death besides the
person who.
I am a witness for the Death of my mother, father and maternal
grandmother. I was not present at my mother's and father's deaths but I
want their deaths to show in my Details; I usually make the spouse a
witness to the death even if the husband drowned at sea so his death
shows in her view.

Unless the registrar is in your data set, why make him a witness? You
can mention who the registrar was in the Tag Memo if you feel his name
should be recorded.
I used to make the minister who performed a baptism or marriage a
witness. later I realized that in most cases that was his only mention
in my data set. I have moved even a minister who is in my data set and
baptized many people form a Tag Witness to the Tag Memo. if I ever want
to find what people he baptized or married ( and I have had no reason to
do so), I can easily search for his name in the Tag memos
