I suggest looking at the Name Tag Entry Screen and making sure that
you have an entry in both the GivenName and Surname fields. It
almost sounds like you may have entered the first name in the
GivenName field, the middle name in the PreSurname field, left the
Surname field blank and entered the last name in the Suffix field (or
some other similar combination) and you have created a Custom Name
Style to accommodate the use of the PreSurname (or whatever) field.

If this doesn't resolve the problem, tell us the entries by field
name in the Name Tag and the Name Style being used.
